Attention Parents!  Changes in Immunization Services

A change has occurred in the process of obtaining immunization services through the local health departments in your area.  In the past, immunization services have been offered on a walk-in basis.  However, due to the closing of two clinics in our district as well as a reduction in staff, it will now be necessary for parents to make an appointment for immunization services.  Making appointments for immunizations will allow the health department staff to be utilized more efficiently and will also help to prevent overcrowding and long wait times during school registration times.

Also, in an effort to expedite services to parents who only need a 121 form, the Department of Health is providing a local phone number that parents may call and request these forms be sent directly to them or to the school.   The phone number to request a 121 form is
(228) 436-6770.
